 To submit your medical expense reimbursement request, please use the Customer Portal.

This platform is optimised for mobile devices, and you can add a shortcut

directly to your phone screen.

 

Request and track the reimbursement of your expenses

Digital submission, no need for original documents: Access the Customer Portal,  and select "Reimbursements > Submit Expenses". Remember to attach the invoice and any additional documents, such as a medical prescription.

Track the status of your reimbursement: To monitor your request, access the Customer Portal,  and select "Reimbursements > Check Reimbursements". In the case of Insured Persons who are minors, the expenses are available in the Primary Member profile.

Certain medical procedures, such as hospitalisation or specific examinations,

require pre-authorisation to be reimbursed or covered.

 

Before undergoing a medical procedure:

Check the need for pre-authorisation:  Log in to the Customer Portal,    and select "Authorisations > Medical procedures requiring authorisation" to confirm if the procedure requires approval.

Request pre-authorisation:  If authorisation is required, fill out the form available in the "Authorisations" section of the Customer Portal and email it to: autorizacoes@future-healthcare.eu

Track your pre-authorisation request:  Check the status of your request in the Customer Portal, click on the icon    and select "Authorisations > Check Authorisations".
